# Data Privacy Statement

wilate® ProCare Patient Support Program microsite — Last updated: \[date of publication\]

## What this statement covers

This statement explains what Octapharma Canada Inc. does with information collected through this microsite. It covers the website only. It does not cover the Program itself, and it does not cover Bayshore's Digital Gateway platform, which has its own privacy policy.

## The most important point

We do not receive patient information through this website. The Enrolment Form is downloaded from this site, completed offline and sent by you directly to Bayshore Specialty Rx Ltd., which administers the Program. Bayshore, not Octapharma, holds and is responsible for enrolled patients' personal health information. What comes back to us is aggregate or de-identified information about how the Program is running, which does not identify any patient.

## What we collect

If you contact us or complete a form on this site, we collect what you choose to give us, which will normally be your name, your professional role, your treatment centre or organisation, and your business contact details.

When the Enrolment Form is downloaded, we can see that a download took place and which centre or organisation it came from. We cannot see anything written on the form afterwards, because the form is completed away from this site and goes to Bayshore.

Like most websites, this one also records technical information automatically: your device and browser type, your approximate location derived from your IP address, the pages you view, how you arrived, and how long you stay. Some of this is necessary for the site to work. The rest depends on the choices you make in the cookie banner.

## Cookies and your choices

When you first visit, a banner asks which cookies you are willing to accept. Cookies that are strictly necessary for the site to function are always on. Analytics and other optional cookies are off unless you turn them on, and you can change your choice at any time through the banner or your browser settings. This site uses HubSpot, which places cookies on our behalf.

## Why we use this information

We use it to run and maintain the site, to see which parts of it are useful and improve them, to understand which centres are aware of and using the Program so that we can support them properly, to respond when you contact us, and to meet our legal, regulatory and safety obligations.

If you ask to hear from us, or if we contact you in the course of an existing business relationship, we may send you information by email. Every message will tell you who sent it and give you a simple way to stop receiving them.

## Safety information

If you tell us about a side effect, a suspected reaction or a product quality problem, we are legally required to record and report it. We cannot keep that information confidential and we cannot delete it on request, because it forms part of a regulated safety record. We keep it for as long as the law requires.

## Where the information is held

This site runs on HubSpot, which stores and processes information on servers in the United States. This means the information described above may be held or accessed outside Canada, and while it is there it may be subject to lawful access requests by courts, law enforcement and government authorities in that country under laws that differ from Canadian law. We use contractual and technical safeguards, and we require our providers to protect the information to a standard comparable to our own.

## Who we share it with

We share information with service providers who help us operate the site and communicate with you, and they may only use it to do that work for us. We may share it within the Octapharma group for the purposes described here. We disclose information to regulators or authorities where the law requires it, including for drug safety reporting, and we may transfer it if the business is sold or reorganised. We do not sell your information.

## How long we keep it

We keep information only as long as we need it for the purposes above or as long as the law requires, and then we delete it or make it anonymous. Website analytics are kept in line with the retention periods set in our cookie settings.

## Keeping it secure

We use physical, technical and organisational safeguards to protect information against loss, theft and unauthorised access, and we review them regularly. No system is completely secure, so please do not send us sensitive information by ordinary email.

## Your rights

You can ask us what personal information we hold about you, ask for a copy, ask us to correct it if it is wrong, and withdraw your consent to any use that depends on consent, subject to legal and contractual limits. Write to our Data Protection Coordinator at Octapharma Canada Inc., 25 King Street West, Toronto, Ontario M5L 1G1.

If you are not satisfied with our response, you can complain to the Office of the Privacy Commissioner of Canada, or to the privacy regulator in your province where one exists. If your question concerns a patient's Program information, that is held by Bayshore, and Bayshore should be contacted directly at 1-844-453-1893 or [wilate@bayshore.ca](mailto:wilate@bayshore.ca).

## Who this site is for

This site is intended for licensed healthcare professionals in Canada. It is not intended for patients, caregivers, members of the public or children, and we do not knowingly collect information from them.

## Changes

We may update this statement. The current version is always posted here with the date it took effect.

Important Safety Information

#### Indications

##### Von Willebrand disease (VWD)

Wilate® is indicated for:

- Treatment and prophylaxis of spontaneous and trauma-induced bleeds in all types of VWD in adult and pediatric patients where use of DDAVP (1-deamino-8-D-arginine vasopressin/desmopressin) treatment is ineffective or contra-indicated.
Clinical data on controlling severe spontaneous bleeding are limited.
- Prevention and treatment of bleeding during and after surgical procedures.

##### Hemophilia A

Wilate® is indicated for:

- Treatment and prophylaxis of bleeding in patients with hemophilia A (congenital or acquired FVIII deficiency) and for the prevention and treatment of bleeding in minor surgical procedures. Clinical trials to evaluate the safety and efficacy of Wilate® in major surgeries are ongoing. Therefore, limited data are presently available on which to evaluate or to base dosing recommendations. Thus, in the case of major surgical interventions, a precise monitoring of the substitution therapy by means of coagulation analysis (FVIII:C) is indispensable.

#### Contraindications

Wilate® is contraindicated for patients who are hypersensitive to this drug or to any ingredient in the formulation, including any non-medicinal ingredient, or component of the container.
