This microsite is owned and operated by Octapharma Canada Inc. ("Octapharma", "we", "us"), 25 King Street West, Toronto, Ontario M5L 1G1. It describes the wilate® ProCare Patient Support Program (the "Program"), which Octapharma sponsors and which is administered on its behalf by Bayshore Specialty Rx Ltd. ("Bayshore").
This site is intended only for healthcare professionals who are licensed and practising in Canada. It is not intended for patients, caregivers or members of the public, and it is not intended for use outside Canada. The Program is not available in Quebec, and nothing on this site constitutes an offer of the Program to residents of Quebec.
By using this site you confirm that you are a licensed Canadian healthcare professional and that you are accessing the site in that professional capacity.
By downloading the Enrolment Form, or by accessing any part of this site that requires you to confirm your status as a healthcare professional, you accept these Terms of Use and the Data Privacy Statement for this site. If you do not accept them, do not use those parts of the site.
We may change these terms at any time by posting a revised version here. The version in force is the one posted on the date you use the site.
The content of this site is provided for general information only. It does not replace the wilate® Product Monograph, and it is not a substitute for your own clinical judgement or for complete prescribing information. Prescribing decisions, patient selection and clinical care remain entirely your professional responsibility. Always consult the current Product Monograph, available at the link provided on this site, before prescribing.
Descriptions of the Program on this site are summaries and are not a contract, a guarantee of service, or a commitment to any particular level, timing or geographic availability of service. Eligibility, the scope of services provided and the conduct of those services are determined and delivered by Bayshore in accordance with its arrangements with Octapharma and with the authorisation given by each patient.
Octapharma may amend, suspend, restrict or withdraw the Program, or any part of it, at any time and without notice. Enrolment in the Program is available only to patients who have been prescribed wilate® by their treating healthcare professional. The Program is offered as a support service and is not an inducement, reward or condition connected in any way to prescribing, purchasing, recommending or dispensing wilate® or any other product.
Bayshore acts as the administrator of the Program and is the party responsible for collecting, holding and using enrolled patients' personal health information. Octapharma does not receive identifiable patient information through this site or through the Program, and receives only aggregate or de-identified information about Program operation.
Completed Enrolment Forms must be sent to Bayshore at wilate@bayshore.ca. You must not send patient information of any kind to Octapharma, whether through this site, by email or otherwise. If you do so, we will not use it for Program purposes and will take reasonable steps to delete or return it, subject to any legal obligation we have to retain safety information.
Bayshore is an independent contractor. Nothing in these terms creates a partnership, joint venture, agency or employment relationship between Octapharma and Bayshore, or between Octapharma and you.
Before submitting an Enrolment Form you must obtain the written authorisation of the patient, or of the patient's legally authorised representative, for the collection, use and disclosure of their personal health information for the purposes of the Program. You are responsible for meeting your own obligations under the health privacy legislation and professional standards that apply to you.
You are responsible for the accuracy and completeness of the information you submit, and for transmitting it securely. You must not use this site for any unlawful purpose, must not attempt to gain unauthorised access to it, and must not interfere with its operation or security.
If you become aware of an adverse event, a suspected adverse reaction or a product quality complaint involving wilate®, report it to Octapharma Canada Inc. by sending an email to adr.canada@octapharma.com. Safety information provided to us cannot be treated as confidential, because we are required by law to record it, investigate it and report it to regulatory authorities. This site is not a channel for urgent medical or safety matters.
The Digital Gateway platform is owned and operated by Bayshore. Creating and using an account on it is governed by Bayshore's own terms and privacy policy, not by these terms, and Octapharma is not responsible for that platform, its content, its availability or its handling of information.

All content on this site, including text, images, layout, trademarks and the wilate® and ProCare names, is owned by Octapharma or its licensors and is protected by law. You may view and print content for your own professional reference and may reproduce the Enrolment Form as needed to enrol patients in the Program. All other reproduction, distribution, modification, framing, embedding or commercial use requires our prior written permission.
This site and its content are provided "as is" and "as available". To the fullest extent permitted by law, we exclude all representations, warranties and conditions of any kind, express or implied, including as to accuracy, completeness, currency, fitness for a particular purpose, uninterrupted or error-free operation, and freedom from viruses or other harmful components.
To the fullest extent permitted by applicable law, neither Octapharma nor its parent, affiliates, directors, officers, employees, agents, contractors, successors or assigns will be liable for any direct, indirect, incidental, special, consequential, punitive or exemplary damages of any kind, including loss of data, loss of profits, business interruption or loss of opportunity, arising from or connected with your access to or use of this site, your inability to use it, your reliance on its content, or your participation in or reliance on the Program, whether the claim is brought in contract, tort, negligence, statute or otherwise, and whether or not we were advised of the possibility of such damages.
Nothing in these terms limits any liability that cannot be limited or excluded under applicable law. This clause does not apply to liability arising from the wilate® product itself, which is governed by the Product Monograph, applicable law and the terms on which the product is supplied.
You agree to indemnify, defend and hold harmless Octapharma, its parent, affiliates, directors, officers, employees, agents, contractors, successors and assigns from and against all claims, demands, proceedings, losses, damages, fines, penalties, costs and expenses, including reasonable legal fees, arising out of or connected with your breach of these terms, your submission of patient information without the required authorisation, your submission of patient information to Octapharma contrary to clause 6, your misuse of this site or its content, or your negligent or unlawful acts or omissions in connection with the Program.
We may change, restrict, suspend or discontinue this site or any part of it at any time without notice, and we are not liable for any consequence of doing so.
Our handling of information collected through this site is described in the Data Privacy Statement for this site. Cookies and similar technologies are used only in accordance with the choices you make in the consent banner.
This site is published in English and in French. In the event of any inconsistency between the two versions, the English version prevails, except where applicable law provides otherwise.
These terms, and any dispute arising out of or in connection with them or with this site, are governed by the laws of the Province of Ontario and the applicable laws of Canada, without regard to conflict of laws rules. You agree to the exclusive jurisdiction of the courts of Ontario.
If any provision of these terms is found unenforceable, the remainder continues in force. Our failure to enforce a provision is not a waiver of it. These terms, together with the Data Privacy Statement for this site, form the entire agreement between you and us regarding your use of this site.
